This change order makes adjustments to unit prices for two existing bid items in the project contract, and also creates three new items for the compensation of work that was not included in the original project scope. The overall cost impact of this change order is estimated increase the amount of the contract by $10,192.93. Thus far, the estimated total impact of change orders on this project contract have increased the amount of the contract by $3,719.89, or slightly less than 0.17% of the total contract value. úÔÉÄÎ×õÜÊÉÔÏÖÊ øÏÖÔÏØØËÔÏÖùØÍÜËÉÐØÏÉ  úÕÜÏÖØîËÙØË  Contractor: Northwest Asphalt, Inc. City Project No.: 2009-1 Address: 1451 Stagecoach Road Shakopee, MN 55379 Contract Name: Industrial Park Area Improvements The City of Hastings and Northwest Asphalt, Inc. hereby agree to this change order to provide compensation for work ordered by the City to correct a design error, to add an additional component to the lighting system, , to install a culvert crossing beneath the new bike trail through the ravine, and to replace a broken water service valve. 1.Correction of Design Error The storm sewer crossing on the south side of the intersection of Spiral Boulevard and Millard Avenue was installed per the plan drawings, yet resulted in the ends of the pipe being located too close to the edge of the roadway. Each end of the pipe had to be extended and the immediate area surrounding the ends of the pipe re- graded. The cost of this work is hereby agreed to as $3,831.87, and a new item in the account records will be created to compensate for this work. It will be described as: CHANGE ORDER #2 - EXTEND STORM SEWER CROSSING. Please see the table on the reverse side for further information. 2.Additional Component for Lighting System The City has requested that grounding rods be provided on all of the light poles that will be installed and reinstalled on this project. Grounding rods were not originally specified in the project documents, and the original bid price reflects their absence. The unit prices will be adjusted upward to reflect the difference in cost for providing the ground rods for each pole. This amount computes to $44.00 per pole. With these changes, the City and Northwest Asphalt agree that the price for Item 107 – NEW POLE & FIXTURE, and item 108 – RELOCATE EXISTING POLE W/RETROFIT FIXTURE, will be increased by $44.00/EA. Please see the table on the reverse side for further information. 3.New Culvert Beneath Ravine Bike Trail After performing field observations, the City determined and requested that a 15” HDPE Culvert needed to be installed beneath a section of the trail in the ravine. This culvert was not contained in the original plan drawings. As there are bid items in the contract for 15” HDPE and for Rip Rap, the installation of these items will be compensated at the unit prices in the bid. However, there is not a bid item for the furnishing and installation of the Flared End Sections on the ends of this culvert. The cost of the installation of the FESs is hereby agreed to as $1,824.00, and a new item in the account records will be created to compensate for this work: It will be described as: CHANGE ORDER #2 – F & I TRAIL CULVERT FES. Please see the table on the reverse side for further information. 4.Water Service Valve Replacement During the operation of a water service valve for the Imperial Countertops property, the existing 4” gate valve was broken by City operators. It was necessary to replace this valve. As the location of the valve was within the parking area for the Imperial Countertops facility, replacement of the pavement after the valve replacement work was also necessary. Paving costs are agreed to be paid at existing unit prices for paving driveways. The cost of the valve replacement work is hereby agreed to as $2,953.06 and a new item in the account records will be created to compensate for this work.
