Flexible Spending Accounts
Flexible Spending Account enrollment
(yellow) forms for pre-tax Medical and
Dependent Care expenses (not for Insurance Premiums) for
2005 should be returned to Lynne Benson in Finance by
December 3, 2004. Please remember that you will not be
automatically enrolled in the plan for 2005 if you were enrolled
in 2004. Feel free to contact Lynne if you have questions or
would like additional information on the plans.
Floating Holidays and Police Holiday Hours
Please remember to use your 2004 Floating Holidays and Police
Holiday hours before year-end, or they are lost. The hours
remaining are listed on your Pay Check Stub or Notice of Deposit.

W-4 (Employee's Withholding Allowance Certificate)
Please remember to turn in a new W-4 if your marital status or
number of allowances has changed. If you are not certain what
you are currently claiming, it is shown on your pay stub or Notice
of Deposit.
For example, on the line for your Federal and State withholding
it will say m-3 if you are claiming married with 3 exemptions
or s-1 if you are claiming single with one exemption, etc. If
you would like a new form or have questions contact Becky or
Lynne in Finance.
Address Check
Employees are reminded to check that Finance has your correct
address on file. Please check the address printed on your
paycheck or Notice of Deposit--this is the address your w-2's
will be mailed to in January.
Please turn all address changes in to Melanie Mesko Lee; your
personnel file will be updated and the information will be
forwarded to Finance for payroll.

2005 Health Insurance Premium Rates
Health insurance premiums are taken via payroll deduction the month prior to coverage.
Therefore, the deductions taken in December will be for coverage in January and will thus
reflect the 2005 health insurance premium rates. In 2005, rates will increase 15%.
